The Family Hope Connection is a partner in the Pennsylvania Statewide Adoption & Permanency Network (SWAN), which serves children in the custody of county children and youth agencies. As a partner in SWAN, we work tirelessly to gather the child’s history, prepare them for permanency and (as appropriate) place waiting children in permanent, loving homes.
