Post-Adoption Services

Family Hope Connection staff is able to offer qualifying post-adoption services free of charge through the Statewide Adoption & Permanency Network (SWAN) to anyone in Pennsylvania who has finalized an adoption. The Family Hope Connection does NOT have to have handled your adoption and you may have adopted domestically, internationally or through a SWAN affiliate.

Services are either provided by FHC or through referral to other resources.

How We Help

• Case advocacy/case management services are provided to ensure you receive access to services required to meet the needs of your adoptive child.
• Counseling and support group support are provided through FHC and referral to other resources.
• We can provide referral for respite care, as requested.

Meet the People We Help

• Five years ago, Yvonne adopted her now 14-year-old daughter, Tina. As Tina entered her teen years, she started behaving in ways, like sneaking out at night, which concerned Yvonne. So, Yvonne reached out to the FHC case worker who had originally helped her through the adoption process for more help.

• Gary and Mary Anne started having trouble with their adopted three-year old daughter, Leigh, around six months after they brought her home from her native China. Leigh threw horrible temper tantrums, frequently sulked for hours, even days, when she didn’t get her way, and seemed to be having trouble sleeping. The couple never experienced any of this with their biological eight-year-old daughter, so they were at a loss for what to do.

• Ben and Olivia adopted Crystal, a three-year-old Chinese girl who had a significant physical disability that required multiple surgeries and slowed her development. As first-time parents, Ben and Olivia needed extra support to get through the challenging adjustment period. Family Hope Connection helped the family navigate the medical system and helped the three develop an identity as a family.
