Permanency Services
The Family Hope Connection is a partner in the Pennsylvania Statewide Adoption & Permanency Network (SWAN), which serves children in the custody of county children and youth agencies. As a partner in SWAN, we work tirelessly to gather the children’s history, prepare them for permanency and (as appropriate) place waiting children in permanent, loving homes.
Child Preparation
Family Hope Connection staff work with children in the custody of county children and youth agencies to give them the support and tools they need to understand and work through their past losses so they may have a stronger sense of identify and better establish strong, healthy bonds with a permanent family.

Pennsylvania adoption law requires all prospective adoptive families to obtain a thorough assessment/home study conducted by a licensed adoption agency or social worker. The home study also is used by Family Hope Connection and other SWAN partners to match families with waiting children.
